Philip Gaziano, M.D. is Chairman and President/CEO, and he helped develop some of ACA’s services from 1998 to 2010 in Hampden County Physicians Associates, a Massachusetts based multi-specialty physician practice now served by ACA .
ACA Provides: Contracting, Consulting, Care Coordination, Reinsurance, Coding, Clinical Rounding, Provider Educational, Chart Auditing, Risk Adjustment, Compliance, Quality Measures Management, Custom Work Flow Design, and Other Provider MSO Services for Managed Healthcare Delivery.
Commitments and Deliverables
Provide quality, financial and utilization web-based tools
Educate local providers and leaders about the tools, regulations, and operations
Provide care coordination tracking, documentation tools, and support services
Provide data security, DR and data backup
Insure compliance with and education about HIPAA, CMS, and related regulations
Track and report quality and utilization data and trends to local ACO leaders
Organize and supervised care coordination and quality activities
Organize disease, beneficiary, & subgroup-specific disease management
Assist in developing and maintaining a community resources database
Educate about, track and report PCP providers’ ACO activities
Help train, assist, and back-up local medical director
Develop a beneficiary portal and assist in member engagement
Received and report all data to CMS 14. Medical record audits as needed
